We strongly believe in the benefits of regular preventative care, and therefore encourage all of our patients to return for periodic preventive recall visits. At the time of your recall visits, please advise us of any changes in your:

1. Health and/or medications being taken
2. Address or telephone number
3. Marital status
4. Employment and/or insurance coverage
